You should complete and sign at home an Acquaintance Form/Child’s Health History Questionnaire before your child’s first visit. Please bring this form with you and, upon entering our office, let our receptionist know you have arrived.

If you wish us to bill your insurance company, also bring a properly completed and signed Dental Insurance Claim form and a copy of your claim booklet. Our Financial Coordinator will review this information with you.
